This form will help determine whether your project requires submission to the Human Subjects Protection Program.
Remember, it is against Federal regulations to conduct research involving human subjects without prior IRB approval.
- Complete the Worksheet for Determining if Research Involving Human Subjects Requires IRB Review. This can be found at http://www.irb.arizona.edu/. This form will help determine whether your project meets the federal definition of research involving human subjects.
- Submit the completed worksheet via e-mail to Alice Pasvogel (Office of Nursing Research) at pasvogel@nursing.arizona.edu.
- For students, include a copy of the approved proposal.
- For faculty, include a copy of the grant proposal if applicable or a copy of the abstract, purpose, specific aims, and methods.
The Principal Investigator will be notified if additional information is needed.
- The Office of Nursing Research will retain a copy for their records.
