The Office of Nursing reviews all human subjects information to ensure that the rights and welfare of subjects involved in the research are protected. Information about the procedures in the College of Nursing related to specific forms can be found in the menu to the right. Forms are available at the Human Subjects Protection Program website.

Human Subjects Training
CITI human subjects training is required by all research personnel. The training can be obtained at www.citiprogram.org
Those who have not completed the CITI training are required to complete the CITI Basic Course for Social and Behavioral Investigators or for Biomedical Investigators depending on the focus of the research. Once completed, the training is good for 2 years. Those who completed the CITI training 2 years ago are required to update their training by completing the CITI Refresher Course. Do not complete the refresher course if you have not completed the basic course.
If you have completed CITI human subjects training at another institution, you can add the University of Arizona as a Learner Institution. Once you have done this, there may be modules specific to the University of Arizona that you are required to complete.

All signed consent forms are to be stored in the Office of Nursing Research.
