Applications for continuing review will be sent to the Principal Investigator from the Human Subjects Protection Program approximately 30 days prior to the month in which current approval expires. If the application is not received, the Continuing Review Form can be found at the Human Subjects Protection Program website (www.irb.arizona.edu).
- Complete the Continuing Review Form and the Continuing Review Report. Further information about the Continuing Review report can be obtained at the Human Subjects Protection Program website (www.irb.arizona.edu).
- Submit the required documents via e-mail to Alice Pasvogel (Office of Nursing Research) at pasvogel@nursing.arizona.edu.
- The Office of Nursing Research will review the material.
- The Office of Nursing Research will obtain the signature of the Departmental Review Chair and send the documents to the University of Arizona Human Subjects Protection Program. It is important to allow sufficient time for the Office of Nursing Research to obtain the required signature (3 working days).
- The PI will be notified of new approval dates for the project. Consenting document(s) will be returned to the Principal Investigator with an IRB approval stamp which will indicate the new approval/expiration dates.
If re-approval is not obtained prior to the expiration date, all study activity (including data analysis) must stop. Please see www.irb.arizona.edu for additional information
If Continuing Review documents are not submitted to the Human Subjects Protection Program prior to the expiration date, the project will be administratively closed by the Human Subjects Protection Program.
Only consenting documents which bear a valid IRB approval stamp can be used for enrollment of research subjects.
Re-approval is required at each periodic review.
ALL SIGNED CONSENT FORMS ARE TO BE SUBMITTED TO THE OFFICE OF NURSING RESEARCH (Room 410) FOR STORAGE.
Please Note: The Principal Investigator does not need to wait until the next scheduled review to officially conclude a study. A Continuing Review Form may be submitted whenever study activities are completed. Prior to graduation, Honors, Masters and Doctoral students must submit the required paperwork to conclude the project. |
