If
you have a hold flag on your account, this form cannot be processed/no
courses can be added to your schedule. You can
check to see if you do have a hold flag conflict on your account
through Student
Link. If you do: call the
Bursars Office at 621-3232.
By
adding this course there will be additional charges/late fees charged
to your account with the University of Arizona
Bursars Office. You can access your account
information through Student
Link or RSVP.
DEADLINE TO PAY FOR SIGNING UP FOR CLASSES WITHOUT LATE CHARGES

This form must be complete to process your
request. This form will go to the identified faculty for their approval
and
you will recieve e-mail notification regarding final
approval for the course.
